2026 Medicare Parts A & B Premiums and Deductibles

The Centers for Medicare & Medicaid Services (CMS) released the official 2026 premiums and deductibles for Medicare Parts A and B on November 14, 2025. After a delay due to the government shutdown, the numbers are now final, and they're higher than 2025 but slightly lower than initial projections.

2026 Medicare Costs

Part B:

  • Standard monthly premium: $202.90 (up $17.90 from $185 in 2025)
  • Annual deductible: $283 (up $26 from $257 in 2025)

Part A:

  • Inpatient hospital deductible: $1,736 (up $60 from $1,676 in 2025)
  • Daily coinsurance days 61-90: $434 (up $15 from $419 in 2025)
  • Lifetime reserve days: $868 (up $30 from $838 in 2025)
  • Skilled nursing facility days 21-100: $217 per day (up $7.50 from $209.50 in 2025)

Part B Premium Details

The Part B premium increase could have been even steeper.

According to CMS, the premium would have been approximately $11 higher per month if the administration hadn't taken action to address unprecedented spending on skin substitutes.

Thanks to changes finalized in the 2026 Physician Fee Schedule, spending on these products is expected to drop by 90% without affecting patient care.

IRMAA Thresholds Adjusted for Inflation

The income thresholds for high-income surcharges increased slightly for 2026. IRMAA now begins at $109,000 for individuals (up from $106,000 in 2025) and $218,000 for couples filing jointly (CMS).

Beneficiaries subject to IRMAA will pay monthly premiums ranging from $284.10 to $689.90, depending on income level.
