1) Agent cannot be previously contracted with Humana through any upline. Cash bonus only applies to new contracts under New Horizons from October 15-December 7.

2) This only applies to brokers who have been newly appointed with New Horizons to sell Humana. Bonus excludes all Medicare Part D/PDP business. Agents will be paid out in a lump sum after January 1, 2021 to ensure the business stays on the books.

3) Agents must order pre-approved marketing pieces from the Humana agent portal. In order to qualify for reimbursement, the agent must submit proof of the marketing pieces ordered as well as a receipt for reimbursement using the form on the New Horizons website. Select any lead-generation marketing piece with Humana branding. Generic pieces are excluded from this incentive. Program period runs from October 15, 2020, through December 7, 2020. Qualifying business must be issued by December 7, 2020. New Horizons Insurance Marketing (NEW HORIZONS) has the exclusive right to change the qualifications and/or rules for this program at any time, including cancellation.
