As you may already be aware, the Centers for Medicare & Medicaid Services (CMS) is readying a fraud prevention initiative that removes Social Security numbers from Medicare cards to help combat identity theft, and safeguard taxpayer dollars. CMS started issuing the new cards in April 2018.All Combined applications including South Carolina now contain an image of the new Medicare Card, and for each applicant, there are now two fields for the Medicare Health Insurance Card Number (this will allow for either or both numbers during the transition period).*Please note: * In addition to the new Medicare Card update, some states have required changes to the health questions. All applications have been updated and are available on the Agent Portal. Please use the new applications as previous versions will no longer be accepted as of June 1, 2018. (*For South Carolina only*: previous versions will no longer be accepted as of June 15, 2018.)The new applications are also on New Horizons website.
