In keeping with our commitment Equitable National Life Insurance Company (Equitable National), an Equitable subsidiary, offers Medicare Supplement plans with competitive rates and is available in the states listed above. For contracting information with Equitable National, please contact 844-502-6708. *Important information * - Equitable will continue to accept new Medicare Supplement applications through November 9, 2018. Applications must be received in good order and must be postmarked no later than November 9, 2018. - After November 9, 2018 new applications for Equitable’s Medicare Supplement in Georgia, Idaho, Illinois, Montana and North Dakota will no longer be accepted. - This action affects new sales only.
